About Service Network

Through our Service Network, we bring together all types of agencies and individuals that serve the Monument Community, Concord CA in the areas of health, social service, education, economic development and housing. Together we partner to create and implement strength-based programs and deliver quality services to Monument residents. The Service Network is about service coordination, communication with residents, and positive community change.

 

The overall aim of the Service Network is to

  1. Create a continuum of accessible, coordinated and integrated services and resource information for Monument families- if a resident has needs they can easily be referred from a variety of locations to a variety of services
  2. Nourish a network of professionals, dedicated to building a healthy and stable community, with a strength based mindset, relevant information, self care, and trainings
  3. Plan and participate in the annual Monument Community Health Fair - 1st Saturday in October
  4. Connect residents with opportunities and activities beyond service delivery, encourage community engagement

 

MCP’s role is to coordinate the Service Network, a generalist bringing specialist agencies together in awareness of and in coordination with each other. Another distinctive piece of the MCP role is that through the MCP Resident Network it is possible to get direct resident input on needs and interests. This information is then shared to the Service Network so that providers adjust programs to meet resident needs. Providers also have a unique ability to get information out about service delivery changes, upcoming events, and other opportunities for residents.
